Forms give you far more flexibility than the single message box you're used to — capture membership requests, detailed enquiry information, or dedicated leads, exactly the way your business needs. You can add a form as an embedded section on any page, or launch it as its own full-page, standalone form.
Creating a Form
Step 1: Open Settings → Forms
Create and manage forms entirely from Settings → Forms in the UENI Editor. There's no inline creation from the page editor — you always start here.
Note: Deleting a form also deletes all of its submissions, and how many live forms you can have depends on your plan (see below) — that's why creation and deletion live in Settings rather than your page editor's Add Section option.
Step 2: Choose a form type
Embedded in a page — placed inline on a page, just like your existing content-block sections.
Single-page (standalone) form — its own dedicated page, activated on your site and with its own URL.
Step 3: Select a premade form or create a blank form
We have created a set of premade forms which you can choose to use as is or as a base to build something different. The premade forms can be modified by adding or removing different fields. If you prefer, you can use the blank form and start from scratch.
The premade types of forms are:
Contact Form - First & Last name, email, phone, message and consent check.
Lead Capture - Name, email, phone and a Qualifying question
Membership Application - Contact details, membership type and a short statement
Quote Request - Name, email, phone and a product/service selection.
Step 4: Name your form
The last step is naming your form; enter the name in the field and then click Create Form. Your form will be added to the list, and it will be ready to use or modify.
Note: This will also be the name of the form on your website; this can be changed later on.
Managing your Forms
Adding a Form to your site
If you created a new form, adding it is very easy by following the steps below:
Head to your Editor and then Pages
Select the Page you want to add the Form
Click Add Section and then select Forms
Select the Form you want to add
The form you selected will be added to your page
Pro Tip: You can add Embedded or Single page forms directly on any page of your website this way, regardless of whether you created them as Single Page or Embedded forms.
Editing your Forms
Once you create a new form, you can edit it from Settings → Forms. Make all major changes to your forms in Settings; for minor or aesthetic changes, you can do them directly on the Page where your Form is.
Changing the Form Title
If you added an Embedded form, you can change the form title after adding it to a page by modifying the Section Title field.
You can change the title of single-page forms both directly on the page where you add the form and in Settings. Go to Settings → Forms, select your single-page Form, and change the Section Title field.
Save your changes, and the title will reflect on your live website.
Page Options
These 3 toggles allow you to transform an existing Form into a single-page form or an embedded form. You can also toggle whether or not you'd like to show the Header and Footer from your standalone page.
More Editing options
Aside from changing the name of your forms, you can control options such as HTML Tags for the title, Font Size and Color, add a subtitle, customize the fields of your form, personalize the submit button, change the alignment of the text on the form, and control the space between fields or the padding at the top and bottom of the form.
Submit Button
Change the button text by entering it in the field; it will default to Send Message.
Change the button alignment on the page; you can choose from 3 different options: Align to Left or Right, and Align to Center.
Change the button to a full-width button (so it spans fully from left to right of your form)
Change the Button colour by using the slider or entering a specific Hex code for the color.
Form Layout & Formatting
You can tweak the Formatting and Layout of your form by using the controls in this section:
Text Alignment allows you to change your form from Left to Right or Center
Space Between Fields controls the distance between each of the fields on your form.
Padding Top adds space to the top of your form.
Padding Bottom adds space to the bottom of your form.
Max Width controls how narrow or wide your form is on the page.
Fields
Premade forms come with different fields, but these can be edited as you see fit. You can add, edit, or delete fields from your form directly on the Fields menu under your selected form.
Deleting a Field
Deleting a field is as simple as clicking the Edit button in the top right corner of the Fields menu. From here, click the trash can icon next to the field you want to delete, and the field will be removed from your form. Click Save if you want to save your changes.
Moving a Field
With the Edit button, you are able to move the fields of your form around depending on your needs. Click Edit, then drag and drop the Field to where you want it and save your work.
Adding a new Field
Click Add a field to add a new field to your form, choose the field you want to add to your form, set the Label, Placeholder text, and Helper text, select whether the new field will be Full Width or Half Width, and check whether your new field will be required and if you want to show the Label you set for your field to be public or only internal.

Modifying a Field
If you have added a Field, you can modify the Labels, toggles, width, and placeholder text by clicking on the Field inside your form menu:
Once inside the Field you want to edit, modify the Labels and options to what you need and save your changes afterward.
After Submission options
This feature allows you to control what happens after a user submits a form to your website.
You can control:
Confirmation message shown after form submission
Show a button that can keep your customer in this form, open a page URL or open a page on your site
Show a second button that can keep your customer in this form, open a page URL or open a page on your site
Sharing your Form
Single page forms are hosted individually on your website, so they can be shared directly via a Direct Link, WhatsApp, Facebook, or X (formerly Twitter). The share button can be found inside your form in Settings → Forms.
Checking Form Submissions
After your form is live and you start getting your first responses, you'll want to check what your customers submitted. To do this, head to the My Customers option in the main navigation menu of your UENI Hub.
Alternatively, you can also click on the "View My Leads" tile in your UENI Hub:
Once you click here, select Forms, and you will be redirected!
In this menu, click on Forms and you will see all of the submissions to your forms:
Click on the Form Submission you want to review, and you will see the inputs from your customer on the right side of the screen, from here you can take some actions besides reviewing the inputs:
Phone button will open your default calling app and will call the number your customer provided.
Reply to button will open your default mailing app and draft a message to the email address your customer provided.
Request a Review will send a premade email to your customer inviting them to leave a review on your Google Business Profile (no email will be sent if you don't have a verified Google Business Profile connected to your account)
Delete Form Submission will delete the form along with all of its contents.
Checking your remaining forms and storage space
Your website can handle limited space and forms depending on your plan. Consult the table below to see your plan and the number of forms and storage space it has.

Deleting a Form
If you run out of storage or forms, you can delete a form to make space for a new one.
WARNING: Deleting a form will delete all of its submissions and file uploads; this action is irreversible.
After you have made sure that you won't need any of the submissions or files from the form you are looking to delete:
Head to Settings → Forms
Select the form you want to delete
Scroll down until you see the Delete option, and click it
Confirm, and your Form, along with all submissions and files, will be deleted
